We can ship your order anywhere in the United States using the shipping method of your choice. When you place an order, we will estimate shipping costs based upon the item(s) in your order, shipping method selected and your shipment’s destination. We provide FREE SHIPPING using USPS Priority Mail for the Alpha-Stim 100, Alpha-Stim SCS and Alpha-Stim PPM units. There is a shipping charge for all accessories via USPS Priorty Mail and for when you request shipment of units or acessories via UPS. Please send us a message using the Contact Us page if you would like to discuss international ordering and shipping options.
Your credit card will be charged with your order is received. However, your order will not be shipped until we receive a prescription from a health care provider (US Only) for the device you ordered. Federal Law (USA only) restricts this device to sale by, or on the order of a licensed health care practitioner. Health care practitioners do not need a prescription. No prescription is required for Alpha-Stim acessories.
You can e-mail or fax us a prescription from your health care provider using the HCP Prescription Form. If you prefer, a licensed health care professional in our network of Alpha-Stim users will provide a prescription for an Alpha-Stim device for you. There is no charge for this service. Please complete the Request for Prescription Form. After you have completed the form, click “Submit” and the completed form will be sent to us via e-mail. You may also e-mail or fax us your completed form requesting a prescription.
